PhilHealth Insurance card – a fully laminated photo ID card that is acknowledged by all government agencies and all other establishments as a valid ID.
To replace a lost ID, submit an affidavit of loss. The notary public usually has a ready-made template for this kind of affidavit. To replace a damaged ID, you just have to surrender your old ID card when you apply for a new one at the PhilHealth office.
Apply as a PhilHealth Member FirstIf you are currently employed in a private company, expect your HR officer to accomplish your PhilHealth membership and provide an ID. Unemployed and self-employed individuals, however, need to register as a “Voluntary Member” to apply and receive an ID from PhilHealth.

Does PhilHealth membership expire? Technically, yes, it does. If you haven't made the required contributions within a specified amount of time, you won't be entitled to claim benefits when you need it.
Registration for a Philhealth membership may be enough to get you covered by their health insurance, but their facilities and benefits may not be easily accessible if you don't have an ID. Being a member and paying your monthly premium does not automatically grant you a Philhealth ID card.

Registration Procedures
- Fill-out two (2) copies of the PhilHealth Member Registration Form.
- Submit PMRF to the nearest LHIO together with the following documents: Two (2) 1x1 latest ID picture, Two valid IDs; and. Any of the following documents: For SSS pensioners.

Both private and government employees must contribute to the Pag-IBIG Fund and PhilHealth. Premium Contributions for Voluntary and Self EmployedSelf-employed members earning a monthly income of P25,000 and below need to pay the contribution of P200 per month or P2400 per year. On the other hand, members having a monthly income above P25,000 need to pay P300 monthly contribution or P3600 yearly.
