NEF files are what is known as RAW files. They store the actual image data as it was captured by the sensor, before any post-processing is applied in camera. If you change the quality settings to be either RAW or JPEG, then only one file will be produced for each image captured.
JPEG fine mode is the JPEG mode with the least file compression, and hence the least amount of compression artifacts (or damage to the image from "lossy" compression.) NEF file format does not use lossy compression, so there is no NEF fine mode. NEF files are the raw data from the CCD.
NEF is an acronym for Nikon Electronic Format. It maintains the exact photographed image with no loss of quality or resolution. Most users working with NEF likely edit and convert them, resulting in a different image file type at the end of the process. This is due to accessibility and editing restrictions of the NEF.
