To cancel your Formstack Account, click on your Profile (upper-right) > Billing > Form Plans. Scroll to the bottom, and you will see a button that says "Deactivate Account".

To link to your Form, go into your Form > Share > Quick Start (or URL Links). Here, you will find the Hosted URL link. This link can be copied and pasted into your website or sent to your end-users via email. On the 'URL Links' page, You will also notice the available QR Code (Quick Response Code) that is generated.
