On your Apple TV, make sure that AirPlay is turned-on in Settings > AirPlay. On your iOS device, go to Settings > Wi-Fi and check that you're on the same Wi-Fi network as your Apple TV. Go to Settings > Network and check that you're on the same Wi-Fi network as your iOS device.
You need an internet connection to set up Apple TV, but not to use it. Obviously, it won't be able to access any on-line content when off-line. You can use peer-to-peer AirPlay to stream from your phone to Apple TV. It uses Bluetooth for discovery, and a point-to-point WiFi connection for data, no network required.
To send and receive streaming audio and video with AirPlay, the devices you're using must first be connected to the same local network. This connection could be wireless, using WiFi or Bluetooth, or wired, using Ethernet. Without AirPort Express, you can use an existing home network setup, such as a wireless router.
Although you can use AirPlay without an Internet connection, it's very buggy. When I stream music without an Internet connection using WiFi AirPlay, I get regular audio dropouts every minute. - Open the Control Center by swiping up from the bottom of the device screen or swiping down from the top right corner of the screen (varies by device and iOS version).
