Central Government Health Scheme (CGHS) beneficiaries and wellness centres can login to online service portal of CGHS, Ministry of Health and Family Welfare to check their CGHS beneficiary and card status.

Loss of CGHS Card
- Copy of FIR from police station.

All government employees are provided with a CGHS plastic card which contains the photo ID and beneficiary ID number. The empanelled private hospitals under CGHS provide treatment to the pensioners on credit /cashless basis for the procedures for which they are empanelled.

Pensioners have an option to get their CGHS pensioner Card either by making CGHS Contribution on an annual basis (12 months Contribution) or by making contribution for 10 years (120 Months Contribution) for getting a Pensioner Card with Life Time Validity.
The claim is to be submitted to the concerned department by serving employees and to the CMO I/C of the CGHS wellness Centre (where the CGHS card is registered) by the pensioner beneficiary within 3 months of discharge from the hospital. The claim is to be submitted in duplicate in the prescribed form.
Serving employees on transfer from one CGHS city to another CGHS city shall submit an application to the Additional Director of the CGHS City (forwarded by the Ministry/ Department from where he /she is being transferred) along with copy of the transfer/ relieving order for transfer of CGHS Cards to another CGHS city

GUIDELINES FOR CGHS BENEFICIARIES:
- Emergency treatment can be taken from hospital in case of no government / recognized hospital located nearby.
- Medicines purchased from open market, in case of OPD treatment taken from recognized private hospital is not reimbursable, and the same is to be obtained from CGHS dispensary.
